To request a Certificate of Insurance, Waiver of Subrogation Endorsement, or Evidence of Coverage Certificate for a client, please complete ALL required fields.

If you are unclear on the information being requested, help text is available by hovering over each field.

Please inform the Client that all documents will be emailed directly to them and the certificate holder.

All Certificates and Waivers of Subrogation will be emailed from "SequoiaOneCertificates@safehold.com" and the email subject line will include: "Sequoia One - Client Name WC Cert/Waiver".
